Care Profiles Development Programme (CPDP)

The CPDP was set up in Autumn 2010 to develop the concept and methodology of care profiles as a commissioning tool.

Successful innovative work by Liverpool Primary Care Trust in 2010 developed care profiles as a commissioning tool for End of Life (EoL) services. The focus was primary and community care, including care homes.

Care profiles do not compete with care pathways – the two are different and complementary.

Care profiles support the specification of service requirements in respect of skill mix, delivery, quality and outcomes at each stage of a care pathway. Such definition and quantification of resources enables clarity, service redesign and the costing of services.

The cross-sector approach is cost effective, flexible and inclusive, and involves personnel from commissioning, providers, GPs, patients and carers. It also supports GP commissioning.
